Concurrent Disorders Service

 

A concurrent disorder is any combination of mental illness and abuse of alcohol or other substances.

Services Provided

Integrated Care Model in major programs (across both Regional Mental Health Care [RMHC] sites)

  • Concurrent Disorder (CD) services are provided to persons aged 16 and older with severe and persistent mental illness and active substance abuse disorder.

  • The key to effective treatment for individuals with a CD is the seamless integration of psychiatric and substance abuse interventions in order to form a cohesive system of care.

  • Upon admission to an RMHC program (e.g., psychosis, mood and anxiety, etc) clients are routinely screened for the presence of a CD. Based on this screening the appropriate care needs are established.

  • Screening for CD is facilitated by the presence of CD Peer Mentors in most short stay units across RMHC. These mentors ensure that CD issues continue to be identified and responded to appropriately. They are linked directly with the CD ambulatory team.

Ambulatory CD Team

Clients who have been identified as having significant CD issues receive treatment from an inter-disciplinary team of:

  • Psychiatrists

  • Nurses

  • Social Worker

  • Psychologist

  • Occupational Therapist

  • Therapeutic Recreationist

With consultation/support from:

  • Spiritual Counselor

  • Dietitian

Assessment

Initial assessments are done by the assessment team with follow-up consultations by a psychiatrist. Individualized treatment is provided through one or more of the following:

  • Outpatient programs

  • Intensive case management for patients with severe and persistent mental illness and substance dependence, both time-limited and ongoing

  • Shared care and telepsychiatry within RMHC, London Health Sciences Centre, and the Southwest Local Health Integration Network through partnerships with community mental health organizations

  • Psychiatric Assessment/Consultation

  • Psychological Assessment and psychotherapy, where indicated

What Can I Expect As a Patient?

Following consultation, the concurrent disorders team will work with you to design a treatment plan that is specific to your needs. Follow-up treatment by our team may be offered.

Alternatively, the team will make specific treatment suggestions and work collaboratively with community service partners in order to provide coordinated services to you. The team also assists in the transition of patients from inpatient services to the community.
 

Referrals

Referrals to Concurrent Disorders Services can be made by physicians. Length of treatment is determined by individual need.
