The
Geriatric Psychiatry Program at
Regional Mental Health Care London is part of an integrated system of
specialized care for the elderly in Southwestern Ontario, offering a full range
of psychiatric services to meet the needs of the patients. A multidisciplinary
team provides assessment, treatment, rehabilitation, support and education for
patients and families.
The Geriatric Psychiatry Program provides
expert care for aging clients/patients who suffer from severe mental illness and
dementia, with a focus on assessment, treatment, rehabilitation, prevention,
family/community support and education. The multidisciplinary team provides a
recovery-based approach to care which will encompass the mental, physical,
spiritual, cultural, psychological and social aspects of aging. The services
provided range from inpatient, outpatient clinics, outreach specialized teams,
and transition support to individuals in long term care hospitals(LTCHs).
Education and support on age-related
psychosocial problems and mental illness is provided to family members, primary
care givers and community health care providers.
The Geriatric Psychiatry Program will be an
integral part of the new specialized mental health facility to be built beside
Parkwood Hospital in London, in keeping with the future vision of mental health
services in Southwestern Ontario.

Southwestern Ontario Regional
Psychogeriatric Program (RPP)
The
Southwestern Ontario Regional Psychogeriatric Program (RPP)
provides home (community and long-term care facilities [LTCFs]) specialized
psychiatric assessments for older adults experiencing mental health problems.
The RPP focus is on assessment, treatment, prevention, support, and education.
The mandate for the program is to build
capacity with local community support services and promote understanding of
mental health needs of the older adult, with a goal of improving quality of
life. This is achieved by providing education to LTCFs, care providers and
linking with existing clinical programs and support services in a coordinated
manner.
Consultative services provide comprehensive
community psychogeriatric assessment and work in a shared-care model with family
physicians and community resources to ensure the recommendations and
interventions are available.

London Health Sciences Centre (LHSC)
Adult
Mental Health - Geriatric Mental Health Outreach Service
As a specialty within the Adult Mental Health
Care Program, London Health Sciences Centre, the Geriatric Mental Health
Outreach Service is comprised of an interdisciplinary team designed to provide
geriatric psychiatry consultation, assessment and treatment to seniors and their
families living in London and Middlesex County.
The goal of the Service is to provide quality
clinical care that recognizes the unique psychiatric and medical needs of older
adults, and education for physicians and professionals training to care for the
elderly. An important component is collaboration with community agencies and
partners in specialized geriatric services.
The Service provides inter-disciplinary
out-patient assessment, treatment and referral for senior citizens who
experience mental disorders of late life, including dementia and related
disorders, mood disorders, psychotic disorders, and substance abuse. Treatments
offered include individual and group psychotherapy, psycho-educational groups
for depression and violence against women, psychiatric care including
pharmacotherapy, family counseling, case management, and nursing and social work
consultation.
Area served: London and Middlesex County
(includes Strathroy, Parkhill, Newbury Four Counties Health Services). Home and
clinic visits are provided but home visits are restricted to the city of London.
Referrals: Although anyone can initiate
a referral, formal agreement by the patient's family physician is required.
Completion of our referral form, including a list of current medications is
requested.

The LHSC Geriatric Consultation/Liaison
Service is an interdisciplinary team that provides consultation to acute
medical inpatients at LHSC who require specialized geriatric consultation. A
component of this consultation is an assessment by a geriatric psychiatrist of
the medical and psychiatric needs of an older adult who may be admitted to
hospital for medical or surgical needs.
